On completion of this form you will be required to submit the following documents to finalize this process and activate your new motor insurance plan :

• Two valid pieces of Identification (Including Valid driver’s license for the class vehicle)
• Proof of address
• Proof of ownership for the vehicle
• Valuation/ Vehicle Inspection
• Completion of our Know Your Client Form (KYC)
